Southington Child Care Registration

HOW TO REGISTER FOR SCHOOL AGE & CLUB Y
(Before & After School program for grades 1-8)
Registrations are done in person at the YMCA Learning Center (YLC), starting April 28th for current participants and their siblings and May 5th for new families.  For your convenience, the School Age and Club Y registration packets are available through our website (www.southington-cheshireymca.org).  The three-page registration packet is screen fill-able.  To download the registration packet, you will need Adobe/Acrobat Reader.  If you do not have this program, you can download it for free on our website or use this “hard copy.”

After going to www.southington-cheshireymca.org, click the Southington Child Care Registration Forms link under Registration near the bottom right of the page.  When the new page appears, you can scroll down to the icons.  To download, simply click the icon titled SA Reg Packet for School Age (grades 1-5) or Club Y Reg Packet for Club Y (grades 6-8).  You can either print the three-page registration packet and complete it by handwriting the information, or you may enter the information by typing the information and using your mouse/pointer and print the packet.  The three-page, screen fill-able packet does not save your information, so please try to complete and print your packet at the same time.  Areas needing a signature are not screen fill-able and require you to sign and date in pen.  This is NOT an online registration and does not guarantee your child a spot in our program.  You must still submit your information in person according to our registration start dates.  Unfortunately, a delay in receiving and printing the registration packet is not an acceptable reason for a delay in submitting and registering your child in the program.  Space is limited, so please plan accordingly.

For School Age and Club Y, the two-page Health Assessment is not screen fill-able, but available for you to download and print.  To download, simply click the icon titled SA Health Record.  This form must be brought to your pediatrician, completed and submitted to the YLC at least two weeks before your child starts our program.
