Southington Child Care Registration

HOW TO REGISTER FOR 2010-2011 NURSERY SCHOOL & HALF-DAY PRE K
Registrations are done in person at the YMCA Learning Center (YLC), starting Jan 11th for current participants and their siblings, Jan 18th for Full and Program Members and Jan 25th for new families/open to the community.  For your convenience, the Nursery School and Half-Day Pre K registration packet is available below.  The three-page registration packet is screen fill-able.  To download the registration packet, you will need Adobe Acrobat Reader. 

To download, simply click the icon titled 2010-2011 NS/HDPK Reg Packet.  You can either print the three-page registration packet and complete it by handwriting the information, or you may enter the information by typing the information and using your mouse/pointer and print the packet.  The three-page, screen fill-able packet may not save your information, so please try to complete and print your packet at the same time.  Areas needing a signature are not screen fill-able and require you to sign and date in pen.  This is NOT an online registration and does not guarantee your child a spot in our program.  You must still submit your information in person according to our registration start dates. Unfortunately, a delay in receiving and printing the registration packet is not an acceptable reason for a delay in submitting and registering your child in the program.  Space is limited, so please plan accordingly.

The two-page Early Childhood Health Assessment is not screen fill-able, but available for you to download and print.  To download, simply click the icon titled EC Health Record.  This form must be brought to your pediatrician, completed and submitted to the YLC on/by Aug 1, 2010.
